1. Have you been granted a divorce decree in Jefferson County Family Court?
2. Has Jefferson County Family Court made a decision on your child custody, support or visitation?
3. Are you asking the Court to review or enforce its decision?
Each form below is available two different ways. If you are working on a computer that has Microsoft Word installed, the Word version should open for printing. (These documents were prepared in Word 2002.)
If this computer does not have a Word programs, you may be able to open and print the .pdf version of the form you need. If your computer will not open the pdf files, you may need to download a free program called Adobe Acrobat Reader.
